Forms Screen
The Forms screen shows all your forms as cards. Each card displays the form title, description, number of fields, submission count, and click count.
Actions on a form card
- Submissions icon — view submissions (or a summary dialog on desktop)
- CSV export — download all submissions as a CSV file
- Edit — modify the form title, description, or fields
- Delete — permanently remove the form
- Favourite — pin the form to the top of the list
- Clicks badge — tap to view analytics
Sharing a form: Each form has a short URL (e.g.
e.sg/contact). Use the share buttons on the card or stats screen to distribute it.Creating a Form
Tap + Add Form to open the form builder.
- Enter a form title (required).
- Add an optional description.
- Tap + Add field to add your first field. A picker lets you choose the field type.
- Fill in the field label. For choice fields (checkboxes, options), add the possible values.
- Toggle Required if respondents must answer this field.
- Repeat steps 3–5 for each field you need.
- Reorder fields by dragging the handle (⠿) on the right.
- Delete a field with the ✕ button.
- Optionally link the form to a Profile.
- Optionally upload a cover image.
- Tap Create Form.
Field Types
The form builder supports 9 field types:

Required vs optional: Mark fields as required to prevent blank submissions. A red asterisk (*) appears next to required fields on the published form.
Viewing Submissions
Tap the submissions icon (📊) on a form card to open the submissions view.
- On mobile, you navigate to a full submissions screen.
- On desktop, a dialog opens showing the submissions summary.
Exporting to CSV
- Tap the download / export icon (⬇) on the form card.
- The app generates a CSV file with one row per submission and one column per field.
- The file download begins automatically (web) or is shared via the native share sheet (mobile).
CSV format: The first row contains field labels as headers. Each subsequent row is one submitted response with a timestamp column.
Form Analytics
Tap the clicks badge on a form card to view analytics (same layout as link stats):
- Total clicks on the form's short URL
- Total submissions received
- 20-day clicks chart
- Last visitors log (IP, country, timestamp)
- Geography map of visitors
